How do I make an appointment with Vigilant Health?

I think I may have diabetes, but I am not a participant through an employer or health plan. Can I still come in for an appointment?

If I schedule an appointment, will you be able to tell me if I have diabetes or pre-diabetes?

Pre-diabetes is an important measure of diabetes because it gives us the opportunity to control the disease before it progresses. We can utilize our unique care model to help you manage pre-diabetes.

How do you work with my primary care doctor?

We maintain close contact with your primary care physician and update them regularly on your progress. This collaboration ensures that there are open lines of communication regarding your care plan, including whatever medications you take and other ailments for which you may be seeking treatment.

My A1c is in the “Caution” category, but I am scared to see a physician. I know diabetes is a hard disease to manage. Can I just wait to see a physician until I get more diabetes symptoms?

No. The longer you leave diabetes untreated, the more severe your symptoms may become. Please schedule an appointment with us as soon as you can to determine the best plan for managing your diabetes.
