Single cases are illustrative for understanding how savings are achieved but can individually be misleading and are of little generalizable relevance to actual savings. Above are actual savings for subspecialty care visits over three years in a $3.6M Plan brought about by Vigilant care management/navigation, as in Example Case 1.

 

Chronic Disease and Clinic Use

Here are savings by the Clinic in care coordination/management of Chronic Disease members over the same period controlled against the members of the same plan with Chronic disease who are not using the Clinic/Vigilant services. (Managed Chronic Conditions are diabetes, hypertension, dyslipidemia, renal failure, stroke, peripheral artery disease, heart disease, congestive heart failure, chronic lung disease/asthma, cancer, mental illness, and pregnancy.)

 

Diabetes and Hypertension Managed Cost per Case

Here are examples of specific cost changes per individual case for Diabetes and Hypertension management by Vigilant in another plan. Cost is the total medical claims paid for an individual with that diagnosis. Note in each case the reduction in cost per case despite the increased number of new cases found by Vigilant.
